Licensed Social Worker

Crossroads Rehabilitation & Nursing is currently looking for a licensed social worker to provide guidance and services to meet the social and/or emotional needs that affect the residents’ ability to achieve their highest level of functionality.

Responsibilities Include:

• Timely complete all documentation related to Social Services, including but not limited to the Comprehensive Care Plan and the Minimum Data Set.
• Develop professional relationships with residents and families as needed for providing guidance on various emotional, social and intellectual needs
• Act as the facility liaison with social, health and community agencies.
• Provide guidance as requested to residents and families in regards to all emotional, mental, environmental and/or physical concerns or limitations.
• Assess, plan and document resident discharge needs in accordance with facility discharge planning policy and regulatory requirements
• Ensure compliance with all HIPAA regulations.
• Follow Residents’ Rights policies at all times.
• Adhere to all facility policies & procedures.
• Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all times

We are a 130 bed skilled nursing facility in Vandalia, OH just north of Dayton, OH. We specialize in long term care and short term rehabilitation services and behavioral health. We have been part of the Vandalia community for over 40 years.
